Semax is a synthetic peptide built from a fragment of ACTH (a hormone involved in your stress response), modified with an added stabilizing sequence to help it last longer in the body. It’s a registered pharmaceutical in Russia and Ukraine, where it’s used clinically for stroke recovery and other circulatory brain disorders — its cognitive-enhancing effects come without the adrenal-stimulating action of the parent hormone.
Semax’s best-documented effect, replicated across multiple animal species and research groups, is a significant increase in BDNF (brain-derived neurotrophic factor) — a protein that drives brain cell survival, the formation of new connections between neurons, and has documented antidepressant effects. It’s also been shown to increase nerve growth factor, relevant to the health of neurons affected in conditions like Alzheimer’s, and to modulate dopamine and serotonin activity in ways connected to motivation and mood.
In Russian clinical use, Semax is used specifically in acute stroke treatment, with human research from that setting showing neuroprotective benefit in stroke, oxygen-deprivation, and traumatic brain injury models. That said, independent replication of these findings outside Russian and Ukrainian research literature has been limited in Western peer-reviewed journals — worth knowing as an honest caveat, even though the underlying BDNF research is well-established.
Because Semax has dopamine-activating effects, evening dosing can interfere with sleep — morning and midday use is generally recommended instead. There’s also a theoretical (not established) concern worth naming honestly: since BDNF promotes cell survival broadly, and chronic BDNF elevation has been implicated in some cancers, this is treated as a caution flag for chronic, long-term use rather than a confirmed risk.
